One phase versus two phase? Mobile versus fixed appliance? With or without extractions? “Reflections on the eternal dilemmas of Class II malocclusion treatment”
Introduction: Class II malocclusion does not indicate a single clinical entity, but rather the result of numerous combinations of different dental, skeletal and muscular factors. Maxillary protrusion, mandibular retrusion or the combination of above may be the underlying skeletal components. Differential diagnosis is mandatory to choose the proper treatment protocol.
When should we start treatment in Class II malocclusion cases? By what means?
The purpose: of this presentation is to describe some clinical solutions adopted in situations in which the prevalence of one or more components of the malocclusion suggested the most suitable choice for the single case.
Material and method: Class II malocclusion cases were chosen to be the presented in this lecture. All patients were both clinically and radiographically examined revealing dental and/or skeletal class II malocclusion. The younger patients received functional treatment by means of either removable or fixed appliance. When functional treatment was not possible orthodontic camouflage of class II malocclusion was performed by means of fixed straight wire mechanics including extractions.
Results: At the end of the treatment all patients revealed correction of class II malocclusion. Achievement of treatment objectives were evaluated by obtaining class I molar and canine relationship, resolution or deep bite and overjet. Significant profile changes were observed in all patients. Post treatment radiologic examination revealed changes either skeletal in those cases were functional treatment was performed.
Conclusion: Numerous treatment modality can be adopted in treatment of class II malocclusion. Despite the large available appliances successful treatment is conditioned in properly selection of each patient’s malocclusion trait. Among advantages of early treatment is the reduced treatment time of the second phase.
